STC EMPLOYMENT & TRAINING SERVICES STUDENT FUNDING APPLICATION
The client agrees to repay any financial assistance received that they are not entitled to, including payments made in error, overpayments, or costs not eligible under this agreement.
Funding is subject to the availability of funds provided by SITAG and Canada. Financial assistance may be reduced or cancelled if funding is changed or discontinued.
If the client stops participating, does not attend regularly, or does not meet minimum participation or progress requirements, funding may be discontinued.
I hereby consent and authorize the release and disclosure to the STC Employment & Training Services any information, reports, materials, or statements, or part thereof, concerning myself as may be required for the purpose of verifying eligibility for financial assistance and checking eligibility for continued assistance. This includes such things as claims for dependents, registration and attendance at an institution, any grade reports, and receipt of all other financial aid.
I hereby authorize the release and disclosure to the STC Employment & Training Services for the purpose of sharing my information, resume, and information for employment purpose only.
I hereby authorize and consent to the use of my voice and/or visual image by STC Employment & Training Services. I give this consent to STC Employment & Training Services to use and publish the same in print and/or electronic formats, with no claim for payment.
I consent to the disclosure and use of my personal information by the Saskatchewan Indian Training Assessment Group, Inc. (SITAG) Network of First Nation Employment Agencies to fulfill the objectives of the First Nation Labour Market Strategy (FNLMS) as follows:
(a) Verifying eligibility for (or entitlement to) benefits through Service Canada under Part II of the Employment Insurance Act and for 1) applying Section 25 referrals, which allows clients who are active EI claimants to continue to receive benefits while taking part in SITAG activities. 2) counsel to leave employment applications if I am already employed.
(b) Permitting SITAG and funders to assess and evaluate the FNLMS.
(c) For registered members of Saskatchewan First Nations: Communicating with the Agency that represents the Tribal Council or First Nation, of which I am a member, for the purposes of collaboration and reporting on services and sponsorships.
(d) Referral to additional support and services through SITAG’s delivery partner, Saskatchewan Indian Institute of Technologies (SIIT). Please mark which referrals you would like by selecting below:
For the purposes of part (a), this consent shall remain in force for one year from this date. This information will be disclosed to the Canadian government and other funders and will be retained by SITAG (100 – 103A Packham Avenue, Saskatoon, SK). I understand the information collected and disclosed is protected under Canada’s Privacy Act and that I have a right under the Privacy Act to obtain access to the information. I understand that SITAG will use my information to determine my eligibility, and I confirm that such information is true and complete.
